Law enforcement agencies should use this form to request to add, remove or change information in the Officer Disciplinary Records Database. POST reviews all requests and updates the database as necessary. To be considered, the officer must not have been decertified pursuant to M.G.L. c. 6E, § 10.
Individual officers may submit requests on their own behalf by emailing [email protected]. Please include the complaint number, a description of the requested changes, the reason for the request and any relevant documents.
